A large cross-sectional study of 2,385 tourists arriving in Lhasa found that over a third developed acute mountain sickness (AMS) within the first three days — and nearly half of those reported symptoms within the first 12 hours of arrival.

Altitude sickness symptoms

Symptoms in Lhasa typically begin within 6–12 hours of arrival. Understanding the progression from mild to severe can be the difference between a safe trip and a medical emergency.

Recommended acclimatisation for Lhasa

Most travellers fly directly into Lhasa. The sudden altitude gain is the single biggest risk factor. Here is the ideal first few days.

Day 1 — Arrival
Rest. Hydrate. Do nothing strenuous.

Check into your hotel and rest for the remainder of the day. No sightseeing. Drink 3–4 litres of water. Avoid alcohol and sleeping pills. Eat lightly.

Day 2 — Light exploration
Short, flat walks only. Monitor symptoms.

If you feel well, take short walks around your neighbourhood. Avoid climbing stairs quickly or exerting yourself. Visit the Barkhor area on foot — keep it slow. Note any headache or nausea.

Day 3 — Gentle sightseeing
Potala Palace, Jokhang Temple at your own pace.

Most visitors feel significantly better by day 3. The Potala Palace stairs are a genuine test — take them slowly and rest often. If symptoms persist, do not push on to higher elevations.

Day 4+ — Onward travel
Only now consider higher destinations.

Having acclimatised in Lhasa, you can consider Shigatse or Gyantse. Never ascend to a higher sleeping altitude while still experiencing AMS symptoms.

6 ways to reduce your risk in Lhasa

  • 1 Arrive via a mid-altitude stopover. If possible, spend 1–2 nights in Chengdu (506m) or Xining (2,275m) before flying to Lhasa. This gives your body a chance to begin adjusting.
  • 2 Drink water, skip the drinks. Dehydration accelerates AMS. Aim for 3–4 litres of water daily from the moment you land. Avoid alcohol for at least the first 48 hours.
  • 3 Consider Diamox (acetazolamide). This prescription medication can halve your risk of AMS. Start 1–2 days before arrival. Consult your doctor — it's not suitable for everyone, and requires a prescription.
  • 4 Rest on day one, no exceptions. Your body needs 24–48 hours to begin producing more red blood cells. Sightseeing can wait. Every hour of rest on day one reduces your risk meaningfully.
  • 5 Eat lightly and avoid sleeping pills. A heavy meal stresses your system. Sleeping pills can suppress breathing during sleep, worsening oxygen levels — exactly what you don't need at altitude.
  • 6 Know when to descend. If symptoms worsen after 24 hours, or if you develop confusion, loss of coordination, or chest tightness — descend immediately. Altitude kills slowly and then quickly.

Frequently asked questions

Will fitness protect me from altitude sickness in Lhasa?
No — and this surprises many people. Physical fitness has little bearing on your susceptibility to AMS. Your cardiovascular efficiency at sea level does not predict how well your body adjusts to lower oxygen pressure. Elite athletes can be just as affected as sedentary travellers. Prior altitude experience is a much better predictor.
Does the Tibet train help you acclimatise before reaching Lhasa?
Mostly no. The Qinghai-Tibet Railway is pressurised and supplemental oxygen is provided inside the carriages after passing Golmud. So while the journey is scenic, your body is not meaningfully acclimatising during most of the trip. You'll still arrive at Lhasa needing the standard 2–3 days of rest.
How long does altitude sickness last in Lhasa?
For most visitors, mild AMS symptoms resolve within 2–3 days as the body acclimatises. If symptoms are not improving after 48 hours at rest, or if they are getting worse at any point, you should seek medical advice or consider descending.
Should I take Diamox before travelling to Lhasa?
Diamox (acetazolamide) is the most widely studied preventative for AMS and can significantly reduce your risk. However, it requires a prescription, is not suitable for people with sulfa allergies or certain kidney conditions, and has side effects including increased urination and tingling in the hands. Speak to a travel medicine doctor or your GP well before your departure date.
What is the best time of year to visit Lhasa for lower altitude sickness risk?
Altitude sickness risk itself doesn't significantly vary by season. However, visiting during the warmer months (April–October) means less cold stress on the body alongside altitude adjustment. The coldest months (December–February) add an extra physiological burden. Most Tibet tours operate April–November.
